Fieldwork Forms
Before you undertake any fieldwork, please seek advice from your academic supervisor. You must fill out the appropriate notification forms below.
FORM 1. Fieldwork Notification - used to notify / be approved each time a party wishes to carry out fieldwork
FORM 2. Fieldwork Participant - to be completed ONCE by each person (Staff, Student, Visitor, Volunteer) wishing to do fieldwork and to be kept on record for next of kin, contact, medical, qualifications and other useful information.
FORM 3. Fieldwork Risk Assessment - to be completed for each fieldwork activity to indicate risks and methods to offset them. This may be reused for repeated fieldtrips if activities / methods / locations remain similar.
FORM 4. International Fieldwork Notification - used to supplement Form 3 for international fieldwork only.
FORM 5. Course Fieldwork Risk Assessment - used only for course fieldwork, rather than Forms 1 & 3.
